"10 Steps After Diagnosis" is a free, practical guide for young people with disabilities, their families, caregivers, and specialists. Diagnosis often comes suddenly and turns everyday life upside down—information chaos, conflicting institutional requirements, and the feeling that you bear all the responsibility alone. This app transforms this chaos into a clear roadmap.

The content was prepared by the Celując w Przyszłość Foundation based on applicable regulations, practice, and the experiences of people who have walked this path. We guide you calmly, in human language, step by step.

10 STEPS THAT ORGANIZE THE SYSTEM
1. The first days after diagnosis—emotions, psychological support, initial documents.
2. Formalities and disability certification—application, commission, reaction to refusal.
3. Your rights in primary care—choosing a doctor, home visits, patient rights.
4. Crisis assistance—what to do in an emergency.
5. Education and support — WWR, school assessment, individual tutoring.
6. Financial benefits and relief — allowances, pension, leave, subsidies.
7. Rehabilitation, therapy, and palliative care — stays, physiotherapy, National Health Fund reimbursement.
