About this app

Encompass Health inpatient rehabilitation patients and their designated caregivers have the option to use the MyEncompass Health Caregiver application to follow along with a patient’s rehabilitation program. The app shows a patient’s real-time progress toward their goals and an overview of their care plan in a secure manner.

Information in the app is regularly updated by our care teams based on the individual care plan for the patient.

TRACK
- Follow the patient’s stay at our hospital as they make progress toward established goals.
o Track goals established by the patient and the hospital's interdisciplinary care team, including mobility, self-care, cognition and behavior, and daily living skills such as meal preparation and medication management.
o See anticipated discharge date.
o Plan for discharge, with instructions that may include:
 Discharge diagnosis
 Instructions for care from nursing and therapy
 Follow-up appointments
 Medications
 Other pertinent information for care at home

SHARE
- Invite others to follow along as the patient progresses in their rehabilitation stay.
- Allow the patient’s legal representative to share their information with others, through granting access to the application on their behalf.

The patient and those who are granted access to the app will have access up to 14 days after discharge.

Ratings and reviews

The app is useless. No clinical notes from therapists, nurses, doctor or bloodwork results. I am used to having all those things available with Trinity or Epic when I need to check on my family or myself anytime of the day. This app updates once a day and doesn't even have all the therapy my mom is getting entered., so still have to call in where half the time no one is available to speak to me because they are so busy. Disappointing.

Horrible all around. When I tried to send an email to the support email address provided, it gave me a delivery failure message. Encompass centers are spectacular, but whoever does this app don't care about its functionality one bit.

App does not work please do not waste your time like I have. Even if all the information is correct it still gives the same error message as seen in the reviews below. I also spoke to facility about this when I was there and all information in their system is correct so obviously the app just simply does not work. Very bad for patient, caregivers, and family!
