HHAeXchange+ provides a simple, fast, and secure mobile experience to connect caregivers with homecare agencies or self-directed employers. To satisfy the 21st Century Cures Act requirement, this app will only record a caregiver’s location when they clock-in and clock-out.
HHAeXchange+ has three modes:
-- Agency mode, for caregivers employed by homecare agencies
-- Self-Direction mode, for family and friends serving as caregivers
-- Care Insights mode, for payers that need member-related questions answered and recorded
Agency mode for homecare empowers caregivers to:
-- Clock in and clock out with location services or FOB
-- Manage their schedule on-the-go
-- Create unscheduled visits
-- Record plan of care duties
-- Conduct skilled or non-skilled visits
-- View member information like demographics, plan of care tasks, or get directions to the member’s home
-- Communicate with agencies in real-time through the Two-Way Chat Message Center
Self-Direction mode allows caregivers to:
-- Start and end a shift
-- View consumer information
Care Insights mode allows caregivers to:
-- Provide responses to member-related health and environment questions
-- Proactively monitor member health
-- Measure against the progress on a specific plan of care to improve member outcomes
