Transitioning your loved one from the hospital to a nursing home can be overwhelming. The main focus of a nursing home stay should be the successful recovery of your loved one. Family members and caregivers of a patient should not feel the added stress that comes along with this transition.

The simple to use yet powerful app aims to breakdown and simplify the over-all routines and processes of a nursing home. The app will provided a comprehensive checklist that guides and empowers each family member to better care for their loved ones.
Check and Balance
The client-centered checklist when filled up, not only shares the accountability from both facility and caregiver, but also serves as reminder that certain important action items might need some attention. It also has an email function that can be sent directly to other family members (to share responsibility) and the care providers in the nursing home.
Proactive, Real-Time and Push Notifications
The app has a push notification option that reminds families and caregivers of suggested things to do based on what date from admission the patient is currently on. This function assures the family that nothing is missed on their part and that proper care is given to their loved ones.
