SpA has experienced an almost unprecedented onslaught of research and management guidelines over the last decade with the advent of new treatments in the form of anti-TNFs. Unravelling the disease process, including the clinical and imaging features can be unnerving at best. We have sought to provide a solid foundation of imaging findings in this disease, its variable appearance and its mimickers in a clear and concise fashion.
