Built to support TEFCA national interoperability framework, PatientChart lets you retrieve your health records from participating networks across the U.S. after a simple identity verification step powered by Persona.

Once verified, you can view your diagnoses, medications, allergies, immunizations, lab results, clinical notes, and visit summaries in one unified, easy-to-understand timeline.

PatientChart also includes TroveCare, an intelligent health assistant that helps you understand your medical information and ask personalized health questions, privately and securely.

It only summarizes or rephrases information already present in the user’s records.
