La libertà alimentare è un diritto ed un giusto premio per i diabetici di tipo 1 ben compensati.
Tuttavia anche nelle situazioni di buon compenso, la glicemia è legata alla capacità di valutare la dose insulinica necessaria a metabolizzare il pasto che si sta per fare.
Questa stima rappresenta in media oltre il 55% della dose insulinica dei boli, ma nel diabetico ben compensato rappresenta praticamente la totalità della dose dei boli calcolati.
Il calcolo del bolo insulinico è diverso da diabetico a diabetico e si può calcolare usano il rapporto Insulina Carboidrati elaborato in genere dalle dietiste e dal diabetologo, oppure si può dedurre dalla esperienza personale del singolo diabetico sul singolo alimento.
Un diabetico esperto quando guarda un pezzo di pane, non pensa più al suo peso o al suo contenuto di carboidrati, ma "vede" le unità di insulina a lui necessarie per metabolizzarlo.
Questa è la filosofia di Insulin&Food ovvero associare ad ogni alimento, per diverse porzioni dello stesso, la quantità di insulina sia essa derivata dal calcolo del rapporto insulina carboidrati (metodo matematico valido per tutti gli alimenti) che dalla esperienza del diabetico sullo specifico alimento.
In altre parole se la valutazione insulinica fatta usando il rapporto insulina carboidrati non ottiene il risultato atteso in specifici alimenti, il diabetico, in maniera esperienziale, può associare direttamente le unità insuliniche equivalenti per quello specifico alimento.
In questo modo, accanto alla foto quantizzata dell'alimento, compaiono le unità insuliniche equivalenti per il soggetto.
Le insuline introdotte dal diabetico possono essere correlate ai CHO da iniettare prima del pasto e/o a quelle derivate dai lipidi e proteine da iniettare dopo o durante il pasto stesso, secondo le indicazioni del medico.
Cambiando la quantità dell'alimento assunto, la dose insulinica equivalente mostrata viene variata.
Insulin&Food può anche fare la foto di nuovi alimenti non presenti nel data base o cucinati in maniera diversa, potendo associare con metodo solo esperienziale la insulina necessaria alla sua metabolizzazione.
Così facendo il diabetico si può creare un suo personale data base di relazione tra alimenti ed insulina sugli alimenti maggiormente utilizzati.
Insulin&Food non è un Calcolatore di boli che tiene presente anche l'insulin on board o il fattore di correzione, ma è un vocabolario alimentare (un data base) dove al posto della composizione dell'alimento viene indicata la dose insulinica equivalente.
Si tratta di un sistema educativo dove il diabetico registra la sua esperienza di correlazione tra alimento e insulina.
Insulin&Food può essere utilizzato:
• sia come si fa comunemente oggi, ovvero facendo prima del pasto una previsione qualitativa e stima quantitativa di quello che si mangerà per iniettarsi tutta assieme la dose stimata
• oppure si può approcciare la stima della alimentazione in una maniera più sicura facendola man mano che si mangia, piatto per piatto e contemporaneamente iniettandosi
la dose di insulina di ogni alimento.
Questo approccio nuovo evita l'errore di previsione qualitativa e quantitativa di quello che si mangerà in quanto non si tratta più di una previsione ma di una registrazione in tempo reale e assicura la somministrazione di insulina nella maniera più fisiologica possibile, un po' come fa il pancreas che produce insulina man mano che si introduce un alimento. Questo approccio facilmente applicabile nei portatori di pompa, può essere usato anche nei soggetti con terapia insulinica classica frazionando in due la dose complessiva, una all'inizio e una alla fine del pasto.

The Lantus dose is calculated as 0.3 units/kg and the prandial dose of insulin (Apidra, Humalog or Novolog) is calculated as 0.1 units/kg. The correctional scale is weight based using the 1700 rule commonly used with pump patients which determines the average decrease in blood glucose expected by 1 unit of insulin.
This app is indicated for use in patients with Type 1 or Type 2 diabetes 16 years and older weighing > 96 lbs (44kg).
This app CANNOT be used in patients with serum Cr > 1.5 mg/dl (as they may have lower insulin requirements)
To calculate insulin units, enter the value of your current blood sugar and the number of carbohydrates contained in your meal. Press the Calculate button and the actual and rounded insulin units will be displayed at the bottom of the screen.
Insulin units are calculated using the following formula:
((Pre-Meal Blood Sugar - Blood Sugar Goal) / Correction Factor) + (Meal Carbohydrates / Carbohydrate Factor)
I wrote this app after my 8 year old granddaughter was diagnosed with diabetes, with the goal of accuracy and ease of use in mind. The goal was reached because she uses it daily to calculate her own insulin units. The units are double checked by an adult, but the app is simple enough for a child to use.

Insulin Bolus Calculator is a medical mobile application that calculates an estimated insulin bolus with active insulin to support your food intake or to correct a high blood glucose (BG). It is very necessarily for diabetic patient (with diabetes mellitus) on insulin therapy.
Insulin Bolus Calculator is similar to "Bolus Wiard" in Medtronic Paradigm insulin pump, "Bolus Advice Calculator" in Accu-Chek Combo insulin pump, "Insulin Calculator" in OmniPod, Animas, One Touch Ping and Dana Diabecare insulin pump.

On the main screen, select your current level of glucose in the blood. (The measurement you have to get on the meter.).
Then specify the amount of carbohydrates.
The program automatically calculates the recommended dose of insulin, with active insulin.
That would keep the calculated insulin dose and take it into consideration in calculating the active insulin following bolus, click "Accept"
To see the details of the bolus calculation , click "Detalis". On this screen you can specify your insulin dose. This dose will be considered when calculating the active insulin in the future.
