CommonHealth helps you collect and manage personal health data and share it with the health services, organizations, and apps you trust.

CommonHealth now supports SMART Health Card digital vaccine records. NOTE: Paper CDC cards are not supported at this time.

What is a SMART Health Card?
- A SMART Health Card is a digital or printed version of your vaccination history or test results, shared through QR Codes and issued by supported states, pharmacies, and providers listed here: https://www.commonhealth.org/smart-health-cards#link-1
- If you have not been issued a SMART Health Card yet, please watch for news from your state or provider as many will be adding the SMART Health Card option in the coming months.

Samsung Pay and Google Pay users! You can now export your SMART Health Card from CommonHealth to your digital wallet.

CommonHealth is connected to more than 400 data sources and counting (including Mayo Clinic, Cleveland Clinic, and New York-Presbeterian). Once your provider is connected to CommonHealth, you can choose to share your health records and data with apps and services vetted by CommonHealth for security and trustworthiness.

CommonHealth was developed by The Commons Project, a nonprofit public trust that puts privacy, informed consent, and the interests of people above all.

Really helpful and useful app. My main problem is that on each device that I installed this (Android tablet, Android phone, and Fire Tablet) it hangs after the check boxes part, right as it attempts to pull up the numeric keyboard for your PIN. I've had to uninstall, reinstall and reboot the devices just to get past that part. I don't understand why it consistently fails at that part, or why a reinstall and reboot fixes it. Other than that, it's great.

app worked fine before, but yesterday I tried to update my records with the new information from CVS , but when i try scanning the Q code it says it cannot validate the card issuer. I contacted CVS about it and they said it has to be the app. Soo with new technology comes new problems to fix . so until its working again I only give it 1 star .. Fix it ..and I will update my review .accordingly

We apologize for you not being able to scan your CVS QR code. We were able to contact CVS and they have updated their issuer information. You should now be able to scan the code without error. Once again we apologize for any inconvenience this may have caused and are grateful for you identifying the issue so that we could work with CVS to resolve.

Worked before, is broken now. When I scan my QR code it says my SMART card cannot be added because "the issuer cannot be validated" without any helpful options to fix, even though I already used it before to add my CVS SMART card last year. The previous card that's on the app did not auto-update with my latest vaccination record so double error on both counts for them. Just so much worse than Apple's equivalent.
