About this app

Use the Yellow Card app to report suspected safety concerns involving healthcare products, including a side effect with a medicine or an adverse medical device incident.

The Yellow Card scheme is the UK system run by the MHRA* for collecting and monitoring information on suspected adverse reactions to all medicines including vaccines, blood factors and immunoglobulins, herbal medicines and homeopathic remedies, and all medical devices available on the UK market.

The Yellow Card app allows users to:

Report safety concerns about:
> Suspected side effects to a medicine or vaccine (including herbal products and homeopathic remedies)
> Adverse incidents involving a medical device (including software, apps and artificial intelligence)
> Suspected defective medicines (a product that is not working properly or not to the manufacturer’s specifications)
> Fake and/or unauthorised medicines
> Blood products

Create an account:
> View previously submitted reports
> Update your reports
> Create a watchlist for alerts to products of interest
> Track new safety information published by the MHRA
> View numbers of reports received by the MHRA

All reports received through the app or website can help the MHRA identify potential new concerns about a medicine, vaccine or medical device and act as an early warning for further investigation. The MHRA will review the product if necessary, and take action to minimise risk and maximise benefit to the patients.

*The Medicines and Healthcare products Regulatory Agency (MHRA) is an executive Agency of the Department of Health and Social Care; the MHRA protects and promotes public health and patient safety by ensuring that medicines, healthcare products and medical equipment are used safely and meet appropriate standards of safety, quality, performance and effectiveness.

Very poor. Just spent over 30 minutes, at 5.30am reporting my wife's reactions, submitted the report and the app status says 'no reports logged'. Like other users I found the app difficult to navigate. It takes far too long for something which is quite serious. It needs to be much simpler and quicker to use, especially if the user is not tech savvy which I am.

Not for Individuals - very bizarre! When you try to use this app to report medicine side-effects, the very first COMPULSORY question is about who you are. It provides a drop-down list of options. ALL are medical professionals! There is no option for patient/individual ie the poor sod having problems with the med. This is a BIG change for Yellow Card - and not a good one. I know from past experience that a lot of GPs can't be bothered doing it, so to exclude patient reports is madness!

Thanks everyone for the negative reviews on here. I went to the MHRA website where it was RELATIVELY easy to complete the form. I did this on my phone, so I don't understand why MHRA can't just use an app that is little more than a bookmark and allow users to fill in the form online. The calendar issue was not too bad on the web. There were a few issues, like I could not report nausea without selecting what kind of nausea from a list of medical terms I was unfamiliar with.
