Tiffany lops off Spawn's head, only to see it turn into a mass of maggots in her hand -- she's been tricked! Spawn attacks, while the evil creatures in the woods gather to aid him. Together, they reduce the angel to bloody scraps. Spawn returns to New York and meets Cog, who warns Spawn that he must gain control of his costume. Meanwhile, Sam shares with Twitch his dream of opening his own detective agency.
Tremor, thirsting for revenge against the man who ruined his life and killed his family, picks off Twistelli's hit men. Twistelli saves his own life by telling Tremor that his brother still lives. Tremor seeks Spawn's help. Spawn at first refuses, but changes his mind when Tremor brings back one of Twistelli's hit men who's willing to talk. Meanwhile, Terry's illness is getting worse.
In his mansion, Twistelli waits for Tremor to show up,planning to capture him and send him to Sicily to replace the missing Overtkill. When Tremor arrives, he finds his brother acting as Twistelli's bodyguard. Spawn appears and warns Twistelli to stay away from Wanda. Meanwhile, Clown enlists the help of Wynn to bring down Spawn. And Cog wonders if Spawn's costume has overpowered its master.
In his office, Wynn plots against Sam and Twitch, setting up a secret meeting where they are to be eliminated. Meanwhile, Terry has finally linked Wynn to illegal activities, but blacks out due to a mysterious illness. In Rat City, Spawn builds a hideout. Cog wonders who is building it - Spawn or his costume.
Spawn and Terry both battle their personal demons. Spawn's suit has completely taken control of him. He hemorrhages and sprouts growth until he is helped by Cog. Terry blacks out and is struck by a semi-. The accident lands him in the hospital. Cy-Gor is in New York city looking for Spawn.
Terry learns that he will die in two months from a malignant brain tumor. Spawn is torn apart by the belief that Terry's a traitor, but he uses all his power to heal him. This sends Spawn hurtling to Hell's second level, where he is immediately attacked by creatures trying to drain his life-sustaining necroplasm. Then suddenly, memories of past injustices provoke Al into a rage and he rises to fight again.
Spawn freefalls through the levels of Hell where he he undergoes a physical and psychological baptism. Layers of Al Simmons' life are peeled away and his weaknesses are exposed. Finally, Malebolgia rips Al's heart out. Meanwhile, Terry's doctors are still puzzled by his miraculous recovery while Cyan refuses to give up a filthy shoelace she found at the hospital. Elsewhere, Sam shows Twitch their new '55 Chevy car.
Cyan's shoelace is the only physical clue to Terry's mysterious recovery from cancer. Terry's nightmares mirror Al's experiences. Spawn enters Hell's fifth level where he is revered as a returning savior. He is forced into a confrontation with the Savage Dragon and defeats the dragon for the position of the Messiah. The dwellers of Hell turn on both of them and stone them to death - or do they?
Al Simmons is returned to his demonic birthplace where he is tormented by a demon in the likeness of Wanda. When he realizes that Malebolgia has tricked him, he destroys the demon and passes another of the Malebolgia's tests. Malebolgia then agrees to leave Wanda alone in exchange for Al's loyalty and servitude. Spawn then enters Terry's dreams where he suddenly awakens knowing that Al is "alive."
To confirm his dreams of Al, Terry searches the alleys for his dead friend. When Cog intercepts and questions Terry about his motives, he ends up mediating their encounter and saving Terry from Al's jealous rage. After Terry shows Al proof of the case he has against Wynn, Spawn helps Terry on a dangerous mission. Meanwhile, Sam & Twitch wait for the "informant" regarding information regarding information about the Chief Banks - Senator Jennings conspiracy.
When Terry meets with Spawn to plan their mission, he tries to understand his friend's chosen hideout as well as their new-found friendship. When he stumbles onto the worms, Al's reaction confuses even more. Terry and Al carry out their mission to document Wynn's involvement in the arms trade. Upon seeing the evidence, Al sends an explosive message to implicate and endanger Wynn's global network infrastructure.
Major Forsberg, a military strategist, has been kidnapped and imprisoned by Jason Wynn, a key figure in U.S. foreign intelligence. Forsberg had determined the insights of the great conqueror Genghis Kahn and relayed some of this to Wynn. When Forsberg denied having full knowledge, his family is murdered by Wynn. Forsberg believes that Wynn is inhabited by Kahn's spirit. Spawn frees Forsberg from Wynn's prison. Meanwhile, Clown continues to direct Wynn to destroy Spawn and Cy-Gor awaits Spawn's return to Rat City.
Although extremely traumatized, Forsberg, the military strategist, is successfully transported to Spawn's alley for safekeeping until Terry and Al figure out how to use him to their advantage against Wynn. When Cy-Gor makes a surprise attack on Spawn, Forsberg huddles in the background watching the battle. He then returns to reality with some startling information about Cy-Gor's origin and connection to Wynn.
Eddie and Andy, the abused children who killed their father, escape their detention and find their way to New York City in search of their hero, Spawn. There, they are befriended by Snake who offers to employ Eddie to courier illegal packages for him. Snake forces Eddie to lead him to Spawn where a confrontation ensues. Snake is killed and Andy is wounded. After medical treatment, the boys return to the detention center, forever stripped of their innocence.
Spawn pressures Terry into making a final move against Jason Wynn using Major Forsberg's information. Later that day, Cyan appears to be kidnapped by Spawn. Confused and distraught, Terry and Wanda try to understand their relationship with Spawn and explain it to police as they search the alleys for Cyan. Wynn finds out that Terry is leaking information to Wynn's "clients." And a scared and hungry Cyan is forced to color pictures for the real kidnapper - the Clown.
While the police search Spawn's alleys looking for Cyan's kidnapper, Wynn directs a surveillance of his own on Terry with orders to find out why the police are looking for him. Spawn then finds Cyan and the Clown who taunts him into a battle. After Spawn uses up a portion of his energy to destroy the Clown, he brings Cyan to her parents.
After Cyan is returned safely to her parents, Al has a long talk with Granny Blake and as usual, her words of wisdom calm him while creating inner confusion. When he gets back to the alley, the alley bums have rebuilt his throne which seems to hypnotize him as he settles into it. The Clown appears in Spawn's trance and reveals startling facts about his past and future. Meanwhile, Wynn confrims Cy-Gor's remains are safe at a testing facility and orders the disappearance of Terry.
Spawn confronts Jason Wynn only to have his vengeful anger stymied once again. Wynn informs him that if he dies, Terry, Wanda & Cyan will be eliminated. Knowing that he doesn't bluff, Spawn turns it into a catch-22 situation. Later in the alley, chaos erupts when Spawn and Angela are thrown together to discuss the unraveling of the universe. An unknown force then heals Al's face. Meanwhile, Terry is informed about a neighborhood petition to force them out of the area.
A shaken Jason Wynn tells his security forces about a "terrorist threat" on his life and they reinforce the area. Meanwhile, Spawn's face has healed itself. Although startled and confused by this new turn of events, Al forms plans to further intimidate Wynn to ensure Wanda's safety and to win her back. Later, Cog counsels Spawn to get him to reconsider his decisions. Elsewhere in the city, Sam & Twitch are pondering how some of their previous cases seem to be tied together when they receive a hot tip and head to the alley.
By morphing his symbiotic suit to match the soldiers, Spawn infiltrates the compound where Wynn has reinforced security. He successfully severs communication and power systems to cripple the battalions, leaving a terrified Wynn trapped. Spawn threatens Wynn with the promise to undermine his operations until nothing remains of his international empire. In the alley, Sam & Twitch get no information from the bums about Spawn. Cog then fills them in on facts that create more questions.
When Cog promises to answer Sam and Twitch's questions, they follow him into the deepest parts of Rat City. Twitch takes notes as Cog tells the tale of Spawn's confused existence as it relates to them. They view Spawn's throne and hear of his life as it was on earth before he returned as a Hellspawn. At the tour's end, Cog tells the detectives that Spawn has chosen them to help recapture his lost humanity.
Cog agrees to accompany Sam and Twitch back to their office to answer additional questions but vanishes from the moving '55 chevy. Back in Rat City, Al Simmons prepares for his reunion with Wanda like an eager groom. When he discovers that his face has returned to its burned appearance, he vents rage and frustration. Terry is uncooperative when Al wants to continue their plan to bring Wynn down. Meanwhile, Wanda becomes concerned when Cyan flies into a rage when the dog snatches her shoelace & pacifier.
The bums gather to commiserate on their lives and share stories and philosophies about their religious beliefs. Spawn diplomatically joins into the conversation and avoids heating the age old debate of God versus the Devil. Later, in another part of the alley, a gang of bums plot to rob and kill one of their own. When Spawn steps in to question their motives, they open fire on him. He collapses, wondering why his costume isn't defending him against this assault.
Sam and Twitch are called to the home of their client, Mrs. Byrd, whose husband has been murdered. As they drive away, puzzling over the scene of the crime, Cog mysteriously appears in the back seat of the '55 Chevy and asks them to help Spawn. Meanwhile, back in the alley, Spawn's murderer bags up the necroplasm as evidence of his death and takes it to his boss, the Freak. When the bums hear of their protector's demise, the battle for the alley begins.
Sam and Twitch discover the remains of Spawn, crucified and headless in a deep alley. When they attempt to remove the body, they find themselves between warring alley gangs. Just as Sam and Twitch gain control of the angry mass, the Freak takes Twitch hostage. During negotiations over the alley turf and Twitch's life, the worms, maggots, rats and flies revive their necroplasmic master.
Twitch is able to break free from his captor, the Freak, when he is distracted by Spawn's sudden appearance. Then, as Sam, Twitch & Spawn struggle to stop the slaughter in the alley, they feel their curious bond intensifying. Spawn tracks down the Freak, where he is drowned by the worms and insects. Meanwhile, Wanda again witnesses Cyan's psychotic behavior when she loses her soother and necklace out the car window.
While seeking a break from their detective work, Sam and Twitch stumble on to a chaotic scene in a lounge that defies explanation. The bar patrons are fighting violently while appearing to be in a trance-like state. They then observe someone kidnapping one of the girls. Losing the suspect, they later find the girl with her throat ripped out. Meanwhile, as Spawn surveys the aftermath of the alley war, he is attacked by a vampire (Wolfram).
Bootsy steps between Spawn and Wolfram, the vampire, to keep them from destroying each other. When Wolfram mentions a nest of vampires in the area, Spawn goes to investigate just as the police arrive and take Bootsy in for questioning. At the police station, Bootsy passes a cryptic message to Sam and Twitch. Elsewhere in an abandoned warehouse, a child is being tortured. Spawn breaks in, kills the abusers and releases the child who reveals an emblem blazoned across his belly.
Sam & Twitch encounter Cog in their office. Cog leaves them a dossier to help Spawn. Eddie Beckett, who found the bag of necroplasm left by the Freak, is stabbed in an alley. With the aid of the necroplasm, he morphs into the Heap, a ghoulish creature consisting of the rubbish mankind has discarded. As the heat in Manhattan climbs to 130 degrees, the angel Bootsy asks Cog to help Spawn. Meanwhile, Spawn encounters the Heap and is absorbed into the Heap's refuse body!
Sam and Twitch ponder the results of their research on Lt. Colonel Al Simmons. Meanwhile, Spawn continues to battle the Heap until he ends up engulfed by the mass of trash just as Cog and Bootsy arrive to save him. When Bootsy kneels to pray for help, he finds the sigil of the Hellspawn, a token signifying that Spawn is indeed gone. Elsewhere, Spawn falls into a dark space where the former Eddie Beckett awaits for him and points him toward a light source where he is welcomed "home."
Spawn has been thrown into the realm of Greenworld, a third power beyond Heaven and Hell. There he learns that the earth is under siege by man's relentless pollution and overuse of its resources. He is given the task to stop the war between Heaven and Hell in order to preserve Mother Nature. Back in the alleys, Bootsy prays for Spawn's return to earth and is removed from earth for interfering with mortal's lives.
Spawn returns from the Greenworld with his new mission. He spots Granny (Mary) Blake at the tomb of her late husband, Jack. Sensing Spawn's huge weight that he now must bare and the heavy heart he feels for Wanda, she tells Al about Jack. After her story, Spawn takes Granny to the Dead Zone and transforms Granny into her youth with her sight back, then enters Heaven and embraces her husband.
Spawn is led to a former church, now an archive of Hellspawn memorabilia. There, Cog tells Spawn of his own life as a Hellspawn, and how Spawn may break his curse. Meanwhile, detectives Burke and Twitch go looking for Spawn. After finding no sign of Spawn in the alleys, the two are confronted by a stranger. Also, Bobby is in despair, practically at rock bottom. After taking a bottle from a murdered drunk, he asks Heaven's forgiveness.
While Twitch lies in a coma, Sam unknowingly trades favors with Spawn for a miraculous recovery. Sam continues trying to solve the latest, seemingly senseless murders of the homeless but leaves the morgue with more questions than answers after the coroner shows Sam an unusual mark on the latest victim. Meanwhile, Spawn and Cog search a museum of antiquities for ancient answers to the mysteries of the murders and they also find an image of the same mark. Later, Spawn confronts Sam and a recovered Twitch about the results of their detective work even as another body is discovered and this time, itÕs a child.
Detective Silbert interrogates Bobby at police headquarters about his involvement in the recent alley murders. Bobby makes his one phone call to Dr. Sarah Frost and later she and Sam discuss his options. Meanwhile, Spawn ÒfeelsÓ the latest victimÕs death and vows to catch the killer of his people. Later, Sam finds Bobby almost dead near SarahÕs clinic. He breaks into the clinic to get help and finds a shrine of sorts from the killer relating to his victims.
With Spawn's help, Sam and Twitch surmise that Dr. Sarah Frost is the serial killer of the alley inhabitants. Spawn confronts her and witnesses her suicide by pesticides, a victim of her own imagined phobia. But when Sam examines Dr. Frost's body, he finds that now familiar symbol branded on her neck. Later, Sam and Twitch receive a cardboard box with a head in it and a cryptic message written on their office wall.
To everyone in the community, Mark Lucas was a good kid. He was always polite, attended church, did well in school, was quiet and never caused any trouble. But one day he brought a gun to church, shot a few people and later took his own life. Meanwhile, Sam and Twitch try to unravel the mystery of Billy Kincaid's apparent ghostly return and Cog and Spawn do some detective work on their own relating to the strange brands they have seen on the latest victims. Are all the seemingly isolated deaths somehow related?
Billy Kincaid's ghost taunts Spawn about the recent innocent murders. Not only has Spawn been unable to prevent the killing of innocent people, but Billy warns him that things are just getting started. Elsewhere, Twitch's wife and son become hostages on the commuter train. When Spawn intervenes to save the innocent people on the train, the gunman is inadvertently killed and becomes one of Billy Kincaid's ghostly crew. Later, Granny Blake feels Billy's ghostly presence and protectively warns him away from her family.
Spawn approaches Sam & Twitch again about discovering an effective way to stop Billy Kincaid from supplying Hell's army with souls. Meanwhile, a reassigned Jason Wynn hallucinates that he is as powerful, deadly, and ruthless as the legendary Genghis Khan. As he imagines himself "Jason the Conqueror," he opens fire on innocent people in a restaurant. Later, when he returns to reality, he finds himself dirty and disoriented in Spawn's alley where Bobby befriends him.
Twitch puzzles over the case of Billy Kincaid and forms a plan with Spawn to stop Kincaid from possessing innocent people, coercing them to perform unspeakable crimes and then sentencing their souls to Hell. Knowing his family is a target of Kincaid's, Twitch convinces Helen to take the kids away for a while. Later, his fears are confirmed when he and Sam receive a hit list with his family, the Fitzgeralds and Granny Blake named.
Sam and Twitch warn the Fitzgerald family to leave the city for their own safety until they can deal with Billy Kincaid's wrath. Knowing that Billy Kincaid occupies Police Officer Rafferty's body, Spawn, with the help of the alley inhabitants, surround and attempt an exorcism. In the heat of the battle, Twitch shoots Officer Rafferty to his great remorse at shooting an innocent family man.
After Cog is unable to reason with Spawn, the ghost of Al Simmons appears to him and they have a heart-to-heart talk. Spawn's epiphany comes when he kills his own ghost. He then returns to the alleys a changed man refusing to be a pawn in someone else's game. He announces to Cog that he is quitting.
Ethan Crone, accused by his editor of being obsessed with Spawn, interviews people from all walks of life around the city who may have witnessed Spawn and his mysterious ways. When he actually tries to spend the night in the Rat City alleys, Cog tells him that there is no Spawn and there never was: he was simply a ghost story told around the bonfires in the alleys.
A frightened child named Mary tells the story of her life with her mean, abusive stepfather. When Sam and Twitch investigate his untimely death, they rule it an accident even though it seems like they know their red-cloaked informant was meting out his type of justice. Meanwhile, Cogliostro meets with representatives from both Heaven and Hell to discuss the precarious balance of earth matters that has been disturbed with the resignation of their missing pawn. Mr. Hell leaves the meeting with a final threat to Cog that he needs to deliver Spawn to him quickly or else, while Ms. Heaven concedes that perhaps the will of God isn't quite what it used to be.
Spawn metes out justice to a highly respected pillar of the community who secretly enjoys child pornography. He later meets with a representative of Hell who becomes hostile and agitated when Spawn points out to him that without mankind Heaven and Hell are left with nothing to rule, nothing to fight over and essentially nothing to do. The demon appears to admit defeat when Angela appears on the scene.
Spawn finds three nervous young men in an abandoned house with the body of a dead Asian girl. All three tell different stories about her demise and the events leading up to it, but each of their stories point the fingers at each other. Justice is served when Spawn gives the knife and the opportunity to carry out the sentence to one of them. He then takes the unlucky knife-wielder away to carry out his own brand of justice.
Rene, a young runaway, has her bravery and pain tolerance tested in exchange for a place to stay in an abandoned house. Later, Mark, the master of the house, drugs her and begins a sacrificial ceremony when Spawn intervenes. And even though Mark demonstrates a hypnotic power over others he comes in contact with, he wants Spawn's powers. Mark makes Spawn an offer to exchange powers and lives.
Mark continues his attempts to persuade Spawn to exchange lives with him, claiming he is dying and knows he'll go to Hell and therefore, if he were a Spawn he could be a ruler there instead of a bottom-feeder. Spawn takes Mark on a tour of Hell, puts him through a ritualistic test that rids him of his terminal illness and begs him not to pursue Hell. When Mark still insists that he wants to be a ruler in Hell, he "accidentally" falls to his death with no arrangement as a HellSpawn.
Spawn is warned by the ghost of Al Simmons to quit interfering in the lives of others. However, Spawn's sense of justice cannot be squelched when he happens upon a group of 13 aristocrats whose boredom has led them to play a deadly game. As part of a coven, they participate in a sacred pact to hunt, kill, and eat each other. The plan was to continue until only one was left and he would have feasted on the souls of the other 12 and therefore have their collective power. He would then be the escort to a terrible beast called a Urizen.
Wanda takes Cyan to a child psychologist hoping to learn the source of her nightmares and inexplicable behavior. However, that widens the chasm between her and Terry as he disagrees with Wanda that Cyan has a problem and leaves during the ensuing argument. Later, Cyan is visited by a being who presents himself as Terry. She becomes frightened and leads the being to Spawn when she runs to him for protection. Wanda interprets Cyan's screaming as another nightmare and when they both fall asleep, Wanda dreams of Al. Wanda later finds out she's pregnant but wonders how conception occurred.
While Spawn and the ghost of Al Simmons confer in the cemetery, two demons inhabit the bodies of two car accident victims. They walk to the nearest diner where they carry out their earthly mission to kill 13 innocent people and then burn their bodies in a sacrificial circle. Spawn senses the evil vibes and goes to the burning hilltop to investigate. There, he is confronted by the ancient and powerful dark god, Urizen.
The demons' mission to attract Spawn to the area is successful when he arrives to investigate the 13 burning corpses placed in a sacrificial circle as bait for the mighty Urizen. In the ensuing battle, Spawn is badly beaten by the powerful Urizen and left impaled upon a pole where he is soon reclaimed by the Earth. Meanwhile, Wanda awakens from a nightmare about her baby to find Terry missing from their bed. Later, as the demons hasten to engage in some earthly sins before they are returned, Angela catches them and asks for an explanation of their mission.
Angela rescues Spawn from his impaled position and gives him the lowdown on their new mission even after she received orders on to act. Other angels debate the order, saying it is thier duty, their purpose to fight while others claim it is their job to follow orders. Spawn and Angela find Cog and try to pry ancient information out of him, but he is vague and uncooperative. Later, after Spawn and Angela leave Cog, they argue and even get into a bit of a tussle. Angela is surprised to learn that Spawn still hasnÕt figured out his earthly role. Meanwhile, Urizen is leading people to their deaths like a monstrous Pied Piper.
Spawn and Angela question the Earthbound demons, Ab and Zab, about their role in freeing Urizen upon the Earth. When they are unable to get information out of the demons, Spawn takes measures to temporarily contain Urizen while Angela halts an army of angels from going to war on EarthÕs behalf. Meanwhile, Terry and Wanda reconcile when WandaÕs doctor tells her that the babyÕs due date is much sooner than expected and therefore the conception was earlier than they thought. Cyan is haunted by a demon who knows she has a connection to Spawn.
Angela halts an army of angels from going to war on Earth's behalf and then comes to the aid of Spawn in his battle against Urizen. Together, harnessing the elemental power of nature, they return Urizen to an Earth-bound prison. Meanwhile, Terry rushes Wanda to the hospital while Granny urges Cyan to protect the Òsad manÓ by using her mind. After their victory against Urizen, Spawn and Angela question the Earthbound demons Ab and Zab and finaly learn that it was malebolgia who set Urizen upon the Earth.
While Wanda lies close to death, Spawn returns to the eighth level of Hell for a final showdown with a weakened Malebolgia. At the height of the conflict, Angela arrives to join in the battle. After mortally wounding Malebolgia and thinking him dead, Spawn and Angela prepare to escape; however, with his last breath, Malebolgia extracts Angela's lance and impales her with it. In a rage, Spawn decapitates Malebolgia with Angela's sword. Carrying Angela's lifeless body, Spawn delivers her to a host of angels who offer him forgiveness and redemption. He refuses this angelic pardon, but the angel who offers it can yet do one favor for Spawn: She appears in Wanda' hospital room and breathes new life into Wanda. Meanwhile, Spawn finally comes to terms with his place in the world.
Spawn returns to the home of Granny Blake to bid her a final farewell. He explains to Granny his belief that Heaven and Hell are two sides of the same coin and defends his decision to choose neither. Granny, however, will have none of it and refuses to let Al disparage her faith. Cogliostro is not letting Al off the hook, either, and tells him that the choice of what role he wants to take is not his to make; that by slaying Malebolgia, he has become Malebolgia's de facto replacement whether he accepts it or not.
At the end of her mental and emotional rope, Thea has a gift that can also be a curse: She can see when people are about to die. Spawn appears to Thea, but she's convinced he's there to harm her. Hell's opportunistic lords are quick to take advantage of humans in Thea's condition, and a smooth-talking, expensively dressed demon approaches her and reenforces her belief that Spawn is out to get her. The demon convinces Thea to give up her soul to him and then reveals, after it's too late, that Spawn was actually there to help.
Being a man of wealth and taste, Eric is a collector of war memorabilia. As his collection grows, begins to look for one-of-a-kind items that no one else can possess. The clerk in the store is more than happy to show him just such an item; a Nazi lamp with a lampshade of human skin, adorned with a rose tattoo. After making the purchase, Eric goes home to admire his latest acquisition; however, Spawn awaits him, and reveals the secrets behind how the lamp came to be.
