In DeKalb, Illinois, Dragon investigates a deformed child, nicknamed Horridus, in an abandoned basement, and brings her to Chicago, where Chief Banks tries to enlist her into the Police Force. During a visit from Amanda Mills, the grotesque Cesspool attacks the Police Department Headquarters, where he is defeated by Dragon, Dart, and the nearby Ricochet and Barbaric. Banks also tries to enlist Ricochet and Barbaric onto the force, but due to her young age, has to turn Ricochet down, who promptly storms off.
Savage Dragon is out of prison and the Vicious Circle is gunning for him. Malcolm Dragon and his stepsister Angel join forces to keep their father alive against an all-out assault in this movie-length epic. Phil literally explodes all over Niseman and Dragon arrives at the hospital a few minutes too late to witness the birth of his son. Kill-Cat is hospitalized, while Dragon learns the truth about Rapture and Peter Klaptin (who is meanwhile being held captive by a shadowy figure). Dragon then prepares to propose to Rapture, only to discover that his son is dead. In reality, the newborn has been captured by The Covenant of the Sword and was replaced with an artificial "dead" fetus.
Sent to Hell by the Fiend, The Dragon and the costumed adventurer Spawn squabble amongst themselves and do battle with The Fiend herself. Meanwhile, Rapture considers having an abortion, Dale Fisher dumps Alex Wilde, and a recovering Phil Dirt reveals to Howard Niseman that he has AIDS. In Hell, the Fiend sends Spawn to Earth, and sics Overlord on the Dragon.

An encounter with the strange creature called The Maxx, who has made friends with Horridus, sends both he and The Maxx for a bizarre trip through the Dragon's subconscious, or, his "outback." They are retreived once Maxx's companion, Sarah, interrupts them. While the villainous Brainiape tries to rob a bank without the consent of the Vicious Circle, he is stopped by their agent, Powerhouse. Dragon and Rapture both suspect each other of infidelity.
After Dragon defeats Doubleheader, he is appointed leader of a special police unit made up of Dart, Barbaric, Horridus, and Rapture, who is new to the force. While Debbie Harris' mother storms into the Police Department HQ and blames Dragon for her daughter's death, Overlord learns of territorial disputes by The Annihilators, a rival gang to the Vicious Circle. While Rapture and Barbaric train, Dragon rescues Frank Darling from a Vicious Circle agent. Frank then confesses his blackmail situation. So afterwards, Dragon and the aptly named "Freak Force" attack the Annihilators at their secret Headquarters.
While the Police Department searches for the missing Dragon, remaining members of the Vicious Circle argue over leadership rights. She-Dragon applies for and is refused membership in the Police. To prove herself, she attacks and is defeated by Rapture. Elsewhere, Dragon manages to break free of the smokestack he is trapped in.
While Dragon has yet to return from New York, Dart and Star fail to stop a prison break by members of the Vicious Circle, including Cutthroat, Mako, Doubleheader, Lowblow, and Hardware, who is murdered by Cutthroat. Dart and Star are rescued by the youthful Mighty Man, a supposedly dead Superhero of the 1940s. Dragon soon visits Dart in the hospital, while Frank Darling is still blackmailed by Overlord and Skullface. Suddenly, Dragon is attacked by The Fiend, a demonic creature of pure hatred possessing a police officer. The officer is killed, however, and the Fiend leaves his body.
WildStar and his companions wander into the underground homeless freaks, and become their leaders to avoid being killed. While Rapture searches for him, Dragon and his fellow Police officers confront and do battle with WildStar and his new team, during which WildStar has some kind of "out-of-body-experience," where he sees the Dragon in the far future.
The Fiend impales and takes the Dragon to Debbie Harris' cemetery, while Overlord puts down attempts on his life, and the Police learn of Frank Darling's defeat of Skullface. The Dragon is attacked by many corpses brought to life by the Fiend, including that of Debbie Harris.
Brainiape, who is revealed to be Adolph Hiter's brain preserved within the body of a gorilla, is sent on a mission to kill the Dragon by Cyberface, the defacto leader of the Vicious Circle. Cyberface is decomposing, so to save him, Abner Cadaver summons the creatures of the undead, who attack the world-renowned paranormal investigator Hellboy.
The Dragon easily defeats Overlord and discovers that The Fiend is in fact Arnold Dimple, the man who shot Debbie Harris. Bonnie Harris manages to separate from The Fiend as a result, and The Devil appears to take her soul. The Devil is confronted and defeated by God, who sends Harris to eternal damnation and Dragon to Earth.
The Dragon returns from Hell with withered arms. Chris Robinson tells his story to Alex Wilde, including his role as Star. After a date with Dart, Justice is attacked by a jealous Kill Cat. Dart, as a result, breaks several bones in Kill Cat's body. Rapture's unborn child begins to kick, causing her internal damage.
Brainiape causes Hellboy to do battle with The Dragon, and then summons a giant sea monster, which eats them both. Unaffected by the Vampire's bite, Cyberface is only restored to health after Abner Cadaver imbues him with the life force of those around him. Hellboy's government allies kill the sea monster, and the emerging Dragon impales Brainiape's brain with a knife, freeing Hellboy from his mental control.
The police and the super-heroes all recover from the ended gang war, while The Deadly Duo and a team called Brute Force arrive late to end it. While Chelsea Nirvana, alias the new Doctor Nirvana, and the Inhabitor kill her father, the original Doctor Nirvana, Rapture proposes to the Dragon.
A squad of Vicious Circle agents, including DoubleHeader, invade the Police Headquarters Dragon and Cyberface are in. Frank Darling also returns and aids the Dragon and the Police in defeating agents, even though Captain Stewart is killed. Cyberface agrees to testify against Overlord in return for his freedom, and a SWAT team including the Dragon confronts Overlord in his highrise headquarters.
Rendered psychotic by the leech-like creature on the back of his neck, the Dragon goes on a downtown rampage. He combats She-Dragon, Star, and several of the homeless "freaks" such as Beast Boy, and finally, Mace. Meanwhile, Frank Darling is almost killed by Skullface.
As super-villains across Chicago fight over the mantle of leadership of the Vicious Circle, the Dragon, still with his bones broken, is found by a cabbie named Chester. Chris Robinson has become a police officer, and Liberty, Justice, and Freak Force have begun patrolling the streets for the villains running rampant. The Dragon arrives in time to prevent Mako, who has escaped from jail, from killing Alex Wilde and Phil Dirt. Meanwhile, She-Dragon is taken to meet Chelsea Nirvana, who has become Johnny Redbeard, and who has decided to join the battle for the leadership of the Vicious Circle.
As Cyberface escapes from Beast Boy's care, The Dragon, Rapture, and She-Dragon battle the Dragon-Slayer, a robot sent to kill the Dragon by newspaper publisher R. Richard Richards, who maintains a personal hatred for the Dragon. Dragon defeats the Dragon-Slayer, arrests Richards, and returns home to find the Fiend waiting for him.
