舞台は田舎。山勘と玉の夫婦喧嘩から始まる。口論の末に山勘は玉を殴りつける。玉が復讐を考えているところに、娘が奇病にかかった轍之丞の命を受けて腕の立つ医者を探している眉唾と太郎丸が登場する。出会った玉は山勘を「棍棒で思い切りぶん殴らないと、医者であることを認めようとしない」優秀だが変人の医者として紹介する妙案を思いつき、それで殴られた夫への仕返しと考え、眉唾と太郎丸に紹介する。木こり仕事から帰ってきた山感は当然医者ではないと言うが、ひどく殴られるので医者であると言わざるを得なくなる。後に引けなくなった山勘は、何の知識も持たないが医者になりきり、轍之丞に紹介され、菖蒲の治療に当たる。菖蒲の病は実は仮病であることがわかる。それも恋煩いなのだ。相手は誰だろうか。
