Saying "shit happens" indicates events are random, have no meaning and there is no accountability or responsibility. It indicates such events could just as easily happen again and there's nothing we can do about them.

Bull.

This book is about catastrophes and how to avoid them, mitigate their effects and learn from them as seen from the perspective of the Masters of Chaos: United States Army Special Forces (Green Berets). Taking the attitude shit happens is negative and is fatal.

Titanic: Systematic Failure
Kegworth Plane Crash: The Danger of Deferring to Authority and Experts
Little Big Horn: Leadership Failure
New London Schoolhouse Explosion: Lack of Focus
The Donner Party: Social Disintegration
From Tulips to the Housing Bubble: Greed Overwhelms Reality
Apollo 13: Success Snatched from the Jaws of Catastrophe

The bottom line is we can predict and prevent most catastrophes because every one has at least one man made factor, of the 7 cascade events, involved. In other words, we have control over whether shit happens. But it means changing a complacent mindset, getting rid of delusional thinking, and viewing the world around us in a Green Beret way.

Because shit doesn't just happen.

What if US Army Green Berets led by Dave Riley must take on not only criminal drug traffickers but also betray their own government?

A US Army Special Forces team carrying sterilized weapons and unmarked uniforms is on its way to Colombia. The mission: conduct covert, unilateral raids to destroy cocaine processing laboratories.

Of course nothing is as it seems especially once the CIA gets involved. Throw in an Israeli mercenary, betrayal and double-dealing in Washington and the team soon finds itself wondering what the real mission is.

“A scorcher of a novel. Mayer had me hooked from the very first page.” Stephen Coonts

From Publishers Weekly
When Colombia secretly requests U.S. aid against the drug cartel, a Special Forces A-Team becomes the "eyes" guiding gunships and helicopters--the "hammer"--against jungle cocaine-processing labs. Then treachery leaves four Green Berets dead and one in the hands of the "Ring Man," deadliest of the drug lords. Special Forces warrant officer Riley and CIA agent Kate Westland are sent in to kill the Ring Man and rescue the prisoner. Their slim chances sink to near-zero when Washington pulls the plug on the mission. But before his people are abandoned, a veteran Special Forces colonel still has a few cards to play. In his first novel (launching a series), former Green Beret Mayer successfully establishes the nuanced differences among U.S. Special Operations units: rangers, Delta Force and Special Forces. But his story line jumps unconvincingly from covert infiltration mission to secret agent-type assassination to commando raid. Descriptions of the tools of unconventional warfare are static, and characters, with the exception of agent Westland, never achieve full dimension.

From Library Journal
A thriller that delivers in all areas--plot, suspense, authenticity, and pace. Riley, a Special Forces man, is sent to one of a series of invasions of Colombia to destroy cocaine processing plants. After two successes and one failure, he is assigned to terminate a drug lord. His companion is a woman, Westland, a CIA agent. The intrigue and plotting of the military and various government agencies, in the United States and Colombia, and the incredible Colombian corruption are interwoven into a minor masterpiece of technology and suspense. The climax will have the reader yearning for more nails to bite. A first novel by a former Green Beret, this is not to be missed by fans of the genre.

Book 2
Book Two in the Shit Doesn't Just Happen: The Gift of Failure Series by NY Times Bestselling Author, West Point Graduate and former Green Beret, Bob Mayer.

THE CHALLENGER: ORGANIZATIONAL FAILURE
“My God, Thiokol. When do you want me to launch? Next April?” Senior NASA official on a conference call to the manufacturer of the solid boosters, when they recommended on the morning of the launch that it be postponed.

THE SINKING OF THE KURSK
“It's dark here to write, but I'll try by touch. It seems like there are no chances, 10%-20%. Let's hope that at least someone will read this. Hello to everyone. There is no need to despair." Captain Lieutenant Dmitri Kolesnikov, commander 7th Compartment (turbine room) Russian submarine Kursk.

THE SULTANA EXPLOSION
“If we arrive safe at Cairo it would be the greatest trip ever made on the western waters, as there were more people on board than were ever carried on one boat on the Mississippi River!” William J, Gambrel, first clerk & part owner of the steamship Sultana.

PEARL HARBOR
“Should hostilities once break out between Japan and the United States, it would not be enough that we take Guam and the Philippines, nor even Hawaii and San Francisco. To make victory certain, we would have to march into Washington and dictate the terms of peace in the White House. I wonder if our politicians (who speak so lightly of a Japanese-American war) have confidence as to the final outcome and are prepared to make the necessary sacrifices.” Admiral Yamamoto, Commander Japanese Navy. (Note that this quote was used extensively for propaganda purposed by the United States by leaving out the last sentence)

Mulholland & The St. Francis Dam
During the Los Angeles Coroner's Inquest, William Mulholland said, "this inquest is a very painful for me to have to attend but it is the occasion of that is painful. The only ones I envy about this whole thing are the ones who are dead." In later testimony, after responding to a question, he added, "Whether it is good or bad, don't blame anyone else, you just fasten it on me. If there was an error in human judgment, I was the human, I won't try to fasten it on anyone else." William Mulholland, chief engineer, Water Department Los Angeles

THE LAST CZAR
“I am not prepared to be a tsar. I never wanted to become one. I know nothing of the business of ruling." Nicholas II, last Czar of Russia.

ALIVE!
“It was repugnant. Through the eyes of our civilized society it was a disgusting decision. My dignity was on the floor having to grab a piece of my dead friend and eat it in order to survive. 'But then I thought of my mother and wanted to do my best to get back to see her. I swallowed a piece and it was a huge step - after which nothing happened.” Dr. Robert Canessa

Every man-made disaster and catastrophe has at least six Cascade Events leading up to the final event, the catastrophe according to the Rule of Seven.

This is a quick read of the Cascade Events of the second greatest loss of life in the history of California: The St. Francis Dam.

Bob examines the Rule of Seven, and how human error plays a role in many catastrophes. Nothing happens in isolation or as a result of a single event. Thus, by learning from history, we can gain insight into preventing a similar catastrophe in the future. William Mulholland, a self-taught engineer, brought water to Los Angeles, but he also brought a catastrophe in the form of the St. Francis dam which failed, killing hundreds. From the very land the dam was placed on, to its design, to the failure of Mulholland on the very day the dam would collapse when called by the worried dam-keeper, to see the pending catastrophe.

This is one of many catastrophes that Bob Mayer, former Green Beret, West Point Graduate and NY Times Bestselling Author, examines in his series of shorts: The Anatomy of a Catastrophe.
