About this app

When patients come to a doctor's practice, clinic or MVZ, they usually have to fill out and sign a few forms first. With mediDOK eForms you can optimize these processes perfectly and digitize them without media discontinuity!

Your practice's own forms (e.g. declaration of consent to data transmission, IGeL contracts, treatment contracts, etc.) are available on a tablet on the mediDOK eForms app. Your patients can fill out and sign the forms digitally in the practice. Finally, the signed documents are saved in the mediDOK archive.

mediDOK eForms is an add-on product to the mediDOK archive solution. The use of mediDOK eForms requires a connection to an existing mediDOK archive and cannot be operated without a corresponding license. You can obtain further information from a mediDOK sales partner.

If you have any questions, please contact your mediDOK sales partner. The next step is to scan the license QR code for the app. This is done in the app via the 3 dots at the top right via "Update license". The corresponding QR code is displayed in the mediDOK administration.
