Fora Health delivers personalised health monitoring and empowers people to share this information with their care teams and/or clinical researchers. Patients use the Android or iOS mobile app to monitor their health. Care teams or clinical researchers can view their patient’s information in a dedicated portal or through the patient’s Electronic Health Records (EHR). Educational modules support all users. Fora Health is used within both clinical research and care pathways where access to accurate real time health information can help support patient insight and the conversations patient’s have with their care teams.
