Home-Based Care Program for Elderly is an ambitious initiative by the National Health Mission, Madhya Pradesh, aimed at delivering compassionate, comprehensive healthcare services directly to the homes of elderly individuals who are bedridden or home-bound. This mobile application is designed to support frontline health workers—Staff Nurses, CHOs, MRWs, and ASHAs—in screening, monitoring, and providing follow-up care to the elderly population across both rural and urban areas of the state.
India is witnessing a rapid demographic shift with a growing elderly population. Madhya Pradesh alone houses over 71 lakh elderly individuals, with 70% residing in rural areas. The HOPE program aligns with the United Nations Decade of Healthy Aging (2021–2030) and the National Program for Healthcare of the Elderly (NPHCE), ensuring equitable access to essential health services without financial burden.
Key Features of the App:
• Patient Enrollment & Screening: Capture demographic and health data during Primary Health Screening (PHS) visits.
• Follow-Up Scheduling: Plan and record multiple levels of home-based care visits tailored to elderly patient needs.
• Health Monitoring: Record vitals, chronic conditions (e.g., hypertension, diabetes), and mobility status.
• Service Tracking: Log physiotherapy sessions, mental health counseling via Tele-MANAS, and tele-consultations with medical officers.
• Data Reporting: Generate reports on care delivery, patient outcomes, and service coverage across villages and urban wards.
• Role-Based Access: Designed for use by CHOs, Nursing Officers, MRWs, and ASHAs with customized workflows.
• Offline Functionality: Enables data entry during field visits with sync capability when online.
