During the course of treatment, therapists set goals with patients and enter updates about their progress during therapy sessions. Patients and family members receive real-time updates through RehabTracker, involving everyone in the recovery process and increasing the likelihood that patients return home safely.

To be invited to RehabTracker today, contact the clinical staff at your facility. Once you provide your consent to use RehabTracker, you’ll receive an email or text message inviting you to download the app and register for RehabTracker.

Ratings and reviews

If I could give this app a zero, I would. Don't forget your password or you'll never be able to use the app again. If you request a new password, it takes so long for the email with your verification pin to arrive, the app times out. You can't put you verification pin in the password field. So you are in an endless do loop. Just call your loved one, don't bother with the app.

I live in Wisconsin. This app sucks. First, it took forever to log on to see my Dad's progress in Pennsylvania. Second, the therapist entered another pt's info for my Dad. After that happened, the app wouldn't even open. It would always close. I uninstalled the app and re downloaded it. same thing happens. This app have a lot of issues and it needs to be fixed. Itwas nice to think I could see my Dads progress from far away, but this app wont allow that to happen.

I like what the app is for and think it is a great idea, but to be honest the app is complete garbage. It logs me out anytime I click anything, it was unclear on who's email I was supposed to put when setting up the account and now I am set up under my mom's name (she's the patient) and it wont let me change the email to hers and so I can't change my email to mine. It has no options to change anything other than your password.
