THREE CALCULATION METHODS

Dose adjustment using the ICSI Antithrombotic Therapy Supplement (15× multiplier on INR deviation from target range midpoint).

▸ Method B — PSM MOH Malaysia
Adjustment using Malaysia's Pharmaceutical Services Programme guidelines (10× multiplier) with automatic PSM MOH dose-rounding ladder applied.

▸ Method C — Custom Adjustment
Clinician-defined adjustment percentage with a ±15% safety cap, for cases requiring tailored dose changes outside standard protocol.
