“3H P-Guardian” is an app that helps patients and clinical trial subjects keep regular records of treatment and post-medication symptoms.
This record (* 1) is accumulated as PHR (* 2), and medical institutions (healthcare workers, researchers, etc.) who introduced “3H P-Guardian” for the purpose of treatment and clinical trials have symptoms and adverse events. It is possible to monitor (* 3) signs of onset.

・3H P-Guardianを導入した施設に会員登録をすると、登録施設からの質問票が配信されます。
　アプリから施設に登録することもできます。（※１）
・カレンダーで質問票の配信スケジュールを確認できます。
・質問票への回答結果を見ることができます。（※２）
・見守り人(家族等)のメールアドレスを設定すると、質問票への回答結果や質問票への回答忘れなどをメールでお知らせします。（※２）
・日々の体調について記録できます。（※１）
・あなたの手帳にメモを登録できます。（※１）
・相談ルームで相談員に相談をすることができます。（※１）
・コミュニティで会員間・会員と運営間でコメントのやりとりができます。（※１）
・疾患に関する体験談を会員間で共有できます。（※１）
・病院・登録施設から休院日などのお知らせが配信される場合があります。
・身体・疾患情報を閲覧・編集することができます。
