PROFESSIONAL SOAP CHARTING
Write Subjective, Objective, Assessment and Plan for every session, with date, duration and modality. Mark pain, tension and trigger points on front and back body charts directly on the note. Capture intake and health history in versioned forms, so an update never overwrites what a client originally signed, and collect consent signatures on screen.

RECORD INTEGRITY YOUR BOARD WOULD RESPECT
Finalized notes are locked. If you need to correct one, the correction becomes a dated addendum and the original text stays preserved with its timestamps. Every record is hash-chained, so tampering is evident. Pick your state to see plain-language record retention guidance for client files, including minor-client rules, with a conservative adjustable default where no state profile exists yet. PRIVATE BY DESIGN
Your client records are stored only on your device, encrypted with SQLCipher. The app opens behind a PIN with optional fingerprint or face unlock, hides its content in the recent-apps switcher, and asks you to re-authenticate before export, backup or delete. Backups are encrypted files that you control and can restore on a new phone. We run no servers for your data.

WHAT IT DELIBERATELY DOES NOT DO
No online booking, no card payments, no insurance billing, no marketing tools. This is a records app that does one job properly. It is not a medical device, does not diagnose or treat any condition, and does not claim HIPAA compliance; your confidentiality obligations depend on your own practice.
