About this app

Cognitive Assessments is a clinical reference tool designed for healthcare professionals to administer, score, and record four validated cognitive screening instruments at the point of care.

ASSESSMENTS INCLUDED

SLUMS — Saint Louis University Mental Status Exam
A 30-point assessment sensitive to mild cognitive impairment and dementia. Covers orientation, memory, attention, executive function, and language. Scoring is adjusted for level of education.

RUDAS — Rowland Universal Dementia Assessment Scale
A 30-point instrument designed to minimize the effects of cultural learning and language diversity. Assesses memory registration and recall, body orientation, praxis, visuospatial ability, judgement, and language fluency.

MoCA — Montreal Cognitive Assessment
Links directly to the official MoCA website (mocacognition.com) for access to the test and administration instructions in accordance with copyright requirements.

AD8 — AD8 Dementia Screening Interview
An 8-item informant interview that detects early cognitive change across multiple dementing illnesses including Alzheimer disease, vascular dementia, Lewy body dementia, and frontotemporal dementia. Can be administered to an informant or the patient directly.
