About this app

We have created an app that responds to these voices.

・I want to eliminate as much as possible the problems of forgetting to tell someone, not being able to tell someone, or not being able to listen to something during a doctor's appointment.
・I want to write down what was said before I forget so I can refer back to it.
・I want to make it easier to share the details of my medical examination with other people.
・It would be nice to be able to see a list of medical records.
・It would be nice to be able to take notes on how your day is going and important events.
・And...something that is easy to input! Something that is easy to continue!

It's ok not to use all the functions, it's ok to use only the parts you need.
I hope that I can help you get closer to your desires through better communication and visualization of your condition.

Detailed explanation of functions

■Diagnosis/interview recording function

In the columns to write in advance, (to the teacher) ``What to tell/ask'', ``Scale of mental and physical condition'',
In the column to write after the consultation, you can write down what the doctor said and what the doctor didn't tell you.
You can easily enter the scale by tapping.
You can continue to input data in advance while waiting for your doctor's appointment, and input data after the consultation while waiting for your checkout or medicine, so you can continue to do so with less burden.
If you enter the date of your next consultation, you can make a note of it each time you have something to say. Before the consultation, you can review what was not covered in the previous consultation.

■Memo function

You can record your daily progress and events.
It has a tone scale and can be easily input by tapping.

■Displaying records (list/calendar)

Records of medical examinations/interviews and memos can be displayed in both list (home screen) and calendar styles.

■Contact book function

You can compile contact information for related organizations such as hospitals, visiting nursing stations, facilities, and government departments.
You can dial from there.

■Document application deadline notification function

This is a function to prevent forgetting to update.
Items for independence support medical care (psychiatric outpatient medical care), mental health welfare notebook, and disability pension are prepared from the beginning. Enter the expiration date and renewal date. When the update is available, a notification will appear on the home screen until you press the update completion button.
