The team of Changhua Christian Hospital Dementia Shared Care Center worked with Kaohsiung University of Science and Technology to establish the National Institutes of Health Dementia Cloud Passport, referring to the clinical dementia care content recommended by the American Academy of Neurology as the content structure of the cloud passport.
Dementia is a continuous care process that requires joint discussion and cooperation between the case, family members, and multi-professional teams. This passport uploads the medical and nursing information of the case to the cloud platform without risking personal information, so that all parties can view and share decisions in real time. Make care continuous and uninterrupted, achieve whole-person and full-course medical care, and protect every dementia case and caregiver.
