About this app

Telecare is the application that allows chronic patients in home care to collect and communicate vital signs data to treating doctors, continuously and in real time.

ACTIVATED BY THE DOCTOR

Telecare can only be activated on the recommendation of your doctor to participate in a remote health monitoring program. The app offers patients and caregivers a simple and intuitive interface to collect and communicate in real time data relating to:
- Vital parameters (eg blood pressure, temperature ...);
- Adherence to therapies;
- Quality of life questionnaires;

EASY TO USE

The patient and caregiver access through the app and enter the measurements manually or automatically through connected devices. The application is also suitable for all users who are unfamiliar with technological tools.

Main features
- Acquisition of measurements through medical devices that can be associated via bluetooth to smartphones or tablets;
- Manual entry of the parameters detected by means of non-Bluetooth devices;
- Access to the measurement history;
- Reminder and alert system in case of out-of-range parameters detection;
- Validated clinical questionnaires profiled by patient categories;
- Activity diary for the collection of data and general information on one's health status.
