Welcome to your LAMITO connect app!

We are delighted that you have decided to participate in LAMITO. You have already received all the necessary documents for LAMITO from your cardiologist and have confirmed your participation in writing.

LAMITO is a quality assurance measure developed by the German Foundation for the Chronically Ill (DScK) in cooperation with MED Management GmbH. The LAMITO connect app helps you to easily and conveniently document your heart health and well-being as part of modern medical care.

What is LAMITO about?

The goal of LAMITO is to regularly collect information about your quality of life, general well-being, and physical activity. The program is designed for patients with an implanted defibrillator (ICD) receiving telemedicine care. The information from the app is transmitted to your cardiologist, helping them to better assess your health and detect any changes early.

What do I need to do to receive the best possible care?
The questionnaires on quality of life and activity will ask you questions that you should answer honestly. The more accurate your answers, the easier it will be for your cardiologist to understand how you are doing with your implanted defibrillator and your telemedicine care. Your answers will help to identify changes in your health early on. This allows you to actively support your medical care and help your cardiologist make the best treatment decisions for you.

How does the LAMITO connect app support me?

With the LAMITO connect app, you can easily record all information about your well-being digitally – conveniently from home or on the go. Through the app, you will receive a total of eight easy-to-understand questionnaires, the first one shortly after your implantation and then again after 6, 12, and 18 months. Each questionnaire consists of two sections: one about your quality of life and one about your physical activity. You will automatically receive a notification when a new questionnaire is ready for you to complete. Should you happen to forget, your cardiologist will contact you directly. Your answers will be automatically transmitted to your cardiologist; you don't need to do anything. After completing the final questionnaires after 18 months, this part of your health monitoring will be complete.
