*There are few bugs and improvement I would like to suggest; 1. Please add scroll up and scroll down button. 2. When confirming the beneficiary as partially vaccinated, the page should not return to homepage. 3. We also need corection for mistake names. 4. If possible, please give us the authority to remove duplicate forms.
