The application for Ensuring Continuum of Care is a user-friendly mobile application designed to facilitate and streamline the process of tracking and maintaining the continuum of care for patients. The application focuses on monitoring and recording patient screenings, referrals, examinations, and the provision of spectacles or further referrals to higher facility. By enabling data entry at two levels, namely Health and Wellness Centers (HWC) by Community Health Officers (CHO), Primary Health Centers (PHC)/Urban Primary Health Centers (UPHC)/Urban Health Centers (UHC) by Eye Mitra Optician (EMO). the app ensures accurate data collection and enables efficient review and progress tracking
