App doesn't work anymore, gives error (fout bij ophalen gegevens)

Very glitchy. Unable to send a request, unable to upload photo. It is impossible to enter your data, the application keeps giving an error

The app closes when I try to start a report. This has worked in the past but the most recent update made it impossible to use.
