About this app

The ProHerz app provides you with optimal support in the daily management of your heart failure (ICD code I.50). Developed by ProCarement GmbH, the app is certified as a medical product and provisionally listed as a digital health application (DiGA) by the Federal Institute for Drugs and Medical Devices and helps you to better understand your health and manage it independently.

• Document archive: Secure and manage all your medical documents in one place.
• Medical Report: Generate clear reports that you can share with your doctor.

Easily share your data with your doctor in order to plan individual therapy adjustments together. ProHerz also supports you by connecting devices for automated transmission of your measurements to make your everyday life easier.

Note: ProHerz is a valuable supplement to medical treatment, but does not replace a visit to the doctor. Medical decisions should always be made in consultation with your doctor.

Legally insured
1. Install ProHerz and register
2. Get a prescription from the doctor or proof of diagnosis (sick note, doctor's letter, etc.) 3. Send prescription or proof of diagnosis to your health insurance company within 28 days or use our prescription service at [e-mail] or on 09191 950431-4
4. The health insurance company will send you a 16-digit activation code
5. Enter the code in the app under “Activation code”.
