ReferralZen puts your agency's referral cases in the hands of the people who move them: referral representatives, intake, clinical and coverage teams. It is the mobile companion to the ReferralZen referral workspace used by skilled home care agencies.

A referral is never one document. Face sheet, orders, notes, insurance card and discharge summary arrive by fax, email and portal at different times, and several people touch each case. ReferralZen reads each document as it lands, keeps one current picture of the case, states what is missing and shows who it is waiting on. Your team decides what is ready.

What you can do in the app

Cases. See the referrals assigned to you and the ones you follow, with status, open holds and time since receipt. Open a case for the documents and conversation behind the status.

Holds. Each failed check is a hold with its reason, the page it came from and an owner: consult order missing or not a valid HHO, demographics incomplete, insurance card missing, patient not eligible, address outside the service area. A hold blocks acceptance without blocking everything else, and it lifts on its own when the missing piece arrives.

Coverage. See whether the address is inside your service area and whether a nurse is available before you call the facility back.

Conversation. Questions, decisions and follow-ups stay with the case, so intake, clinical, coverage and the rep work from one page instead of a text thread.

Documents. View every page in the case. Photograph a signed order or a face sheet at the facility and upload it; it joins the case and is filed by type.

Search. Find any patient, case or document in your agency by name.

Notifications. Get an alert when a hold lifts, a message arrives or a case is assigned to you. Push notifications carry no patient information; the details wait inside the app.

Built for PHI

ReferralZen is HIPAA-ready and operates a SOC 2-aligned control environment. Data is encrypted in transit and at rest, access is role-based, and every view, edit and acceptance is logged. No patient data is used to train AI models. A business associate agreement is signed with your agency before any PHI moves.

Who it is for

ReferralZen is used by home care agencies. You need an account from your agency to sign in; there is no self-service sign-up in the app, and it is not intended for patients or families. Ask your administrator for access, or visit referralzen.com to learn about ReferralZen for your agency.
