CareBeacon is a HIPAA-aware care coordination platform built for home health and hospice providers, their care teams, patients, and authorized family members. It closes the communication gaps between everyone involved in a patient's care — clinicians, schedulers, administrators, patients themselves, and the family members or healthcare proxies authorized to be part of the conversation.

KEY CAPABILITIES

• Secure messaging — direct and group chats between care team members, patients, and authorized family/POAs, with read receipts, do-not-disturb hours, and presence indicators

• Scheduling and visit coordination — view today's visits, accept assignments, track travel time, and confirm arrivals

• Turn-by-turn navigation — Mapbox-powered routing to patient addresses with live ETA bubbles, off-route detection, and speed-limit awareness

• Voice and video calling — one-tap audio/video calls within the care team, plus multi-party conferencing for IDG meetings

• Electronic Visit Verification (EVV) — location-anchored arrival/departure logging that meets payer EVV requirements

• Symptom reports and daily check-ins — patients log how they're feeling; clinicians see trends and act on them

• Call logs and triage — capture, route, and review intake calls with team handoff

• Family member access — authorized family contacts and POAs get scoped visibility into the patient's care, with role-based controls

WHY CARE TEAMS CHOOSE CAREBEACON

• HIPAA Business Associate Agreement available for healthcare organizations
• Encrypted in transit (TLS) and at rest
• Role-based access controls (clinician, patient, POA, viewer, administrator)
• Audit logging of every access to PHI
• Multi-factor authentication options
• Built for clinical workflows, not retrofitted from a consumer chat app

WHO IT'S FOR

CareBeacon is designed for licensed home health agencies, hospices, palliative care programs, and care management organizations — and is enrolled through the organization, not by individual end users. If you're a patient or family member, your care provider will invite you in once they've signed up CareBeacon for their team.
