In an unexpected twist of fate, April lands in the hospital where her estranged uncle, George (Steven Weber), works as an oncologist, and she learns the devastating news that she has cancer.

A new assignment to profile gubernatorial candidate Bruce Hendrie leads to an unexpected run-in with his son, Leo (Scott Michael Foster). April makes a startling discovery about her family, while Sara struggles with solo parenting.

As things begin to heat up with Dominic, April second-guesses the status of their relationship when she learns he has a reputation as the office playboy. To help take her mind off of Dominic and to start facing the reality of her symptoms, she volunteers for an assignment at work about a supposed cancer-curing miracle juice and finds a confidant and support in the juice maker, Gerald (guest star Sam Anderson). Meanwhile, Sara makes a surprise connection with one of her therapy patients and hosts an awkward dinner for her new boyfriend, Ben, to meet the family.

April finds it harder to juggle the increasing severity of her symptoms with her obligations at work and her attempts to be a supportive daughter and sister. After missing an important appointment, George urges April to get her priorities straight. April also learns more about Leo Hendrie, but Leo jumps to conclusions about April's motives. Meanwhile, much to her own surprise, Brenna bonds with Greer, a popular girl at school.

Dominic takes April to see a psychic for fun but the results disturb both of them, leading Dominic to come clean to April about a family secret. When her own family secrets become too much for her to bear alone, April enlists Dominic's help in learning more about her "sister" Natalie.Meanwhile, April receives an official diagnosis from George and Sara's mounting concern for April's health leads to a confrontation.

April gets a surprising invite to shadow Bruce Hendrie at his campaign ad shoot for the paper. While there, Leo senses her apprehension of her diagnosis and challenges April to take control of her future. Not one to back down from a challenge, April accepts and gets a taste of Leo's reckless and carefree life. But when things go too far, she learns the severity of Leo's illness. Meanwhile, Brenna is unsure of her future with Kiernan, even more so when the relationship causes tension with her new friend Greer.

April sees a fertility doctor to learn about her options prior to starting chemotherapy and looks to her cancer support group for advice on how to tell Dominic about her leukemia. She gets Leo's blessing to write a piece that paints Bruce Hendrie in an unflattering light and wins some hard-earned recognition at work. Meanwhile, George makes an effort to help Sara cope with her emotions and the pressure of caring for April, and Dominic gets a surprising offer which could change everything.

April plays hooky from work to hang out with Beth and folks from her support group, and she uncovers something pleasantly surprising about Leo. Elsewhere, Brenna goes on a class trip to Miami, where Greer helps her meet Natalie.

April prepares for her first chemo treatment and a hospital stay by trying to enjoy a few ordinary days, but that doesn't work out thanks to drama from Leo, Beth and Sara. Elsewhere, Brenna attempts to make time for both Kieran and Greer.

April starts her treatments, but she begins to panic as the chemo and hospital confinement take their toll on her, so she reaches out to someone unexpected. Meanwhile, Dominic finds a changed April when he returns.
