Evil Skylar and the Annihilator plan to take over the hospital and, eventually, the world.

Oliver and Kaz plan to visit the Annihilator's lair to take his collection of superpowers and then restore them to the superheroes.

The boys notice a few heroes displaying bizarre, if not slightly evil behavior, and suspect that there could be a hospital mole that is working with the Annihilator.

Skylar asks Oliver to the school dance while Kaz pretends to be evil to spy on the villains. Meanwhile, Alan gets caught in the middle of a prank war.

Oliver and Kaz travel to Skylar's home planet, Caldera, to search for the Annihilator's former mentor, Hapax the Elder. The boys journey to the highest volcano to defeat a monster.

Skylar and the Annihilator have discovered the boys' plan, but Skylar realizes that her powers are greater than the Annihilator's and no longer wants to take orders.

A man from the future visits the boys and reveals himself as Kaz twenty-five years from now. Future Kaz warns the boys of an evil villain wreaking havoc on the future - Oliver!

Oliver leaves Mighty Med to work on a school group project with Skylar, Alan and Gus. Kaz is left alone to tend to an ailing hero while Oliver spies from a secret spy camera.

Horace lowers the standards required to be a superhero and Kaz holds superhero tryouts. Meanwhile, Oliver takes Skylar to a dance tournament to impress her.

Oliver must care for a sac of highly valuable eggs when a pregnant female superhero is wheeled into the ER. Kaz helps Oliver hide his "pregnant" state at school.
