Sensorium provides data services to detect early health risks and health needs of an individual or group of individuals and monitor their development. With the Sensorium app you can monitor various sets of health aspects in real-time and give the healthcare provider insight into the current health situation and health needs. As soon as Sensorium detects particularities, it automatically makes a risk assessment and provides advice back to the person or group of people. This versatility makes Sensorium unique; it can be used for self-monitoring, remote care, patient panels and population management.

It goes like this. Your healthcare provider will determine which health analysis and / or health needs assessment program is appropriate for you. The healthcare provider does this to help you improve your health and to better tailor its services to the needs of you and those of others, now and in the future. Participation is now only possible at the invitation of your healthcare provider (s). The digital invitation from your healthcare provider contains a unique link that gives access to the program. After completing your registration, you can get started in the Sensorium app. From then on, Sensorium helps you to recognize patterns, identify and stratify risks, classify the population and carry out health interventions on the basis of data analyzes on your collected structured data.

The data that is recorded by or from you is only accessible to you. Your data is also used to support population management. Sensorium processes this data in such a way that it can no longer be traced back to a person.
As soon as you indicate that you no longer want your data to be used for population management, all your data will no longer be part of the population-based analyzes with retroactive effect.
