Docket® is commissioned and directly endorsed by the following official government entities: Alaska Department of Health, Idaho Department of Health and Welfare, Minnesota Department of Health, New Jersey Department of Health, Utah Department of Health and Human Services, and Wyoming Department of Health.

Alaska, Idaho, Minnesota, New Jersey, Utah, and Wyoming residents with a valid phone number and/or email address on file with their state's immunization registry can access digital immunization records with the Docket® app.

Use of Docket® is entirely optional.

This app has a horrible user interface & design, everything it tries to have you do is confusing. Like every step. Asks questions - there is no save option (example:personal info, no save option after updating), buttons cover screen text, picking a date is overly complicated, even on screen instructions are not clear (like update year note, there are 2 yrs on widget). Think I'll just continue to keep a photo of my vaccination card on my phone instead of this app. Don't waste your time.

Awful implementation of a great idea. App has horrible UX engineering. Often difficult to know what is next-- example: at one point it says records are being searched-- to update tap the "refresh" icon, but there is no such icon on the screen. For me, my vaccination records have my non-cell number. App has "voicemail" verification option-- but the automated caller says an 8 digit code so fast it is almost impossible to catch.

The phone of record is a landlines so a voice-mail was requested. The verification code is given so quickly it is difficult to record. Whe I did succeed, the code given was 8 digits, but the verification spot on the app shows only 7 spaces. When I tried to enter, nothing happened. I then tried to setup my cell. Things progressed well until I tried to add the verification code. The keypad flashes on but quickly disappears when I touch the verification line. Finally worked voice-mail.
