"MetaClinic-home" is a preventive telemedicine system based on the monitoring of
some basic parameters and specific questionnaires that, when collected regularly by the user at
home, can provide
a timely alert on the worsening of chronic diseases already diagnosed and
treated in the same specialist centers that prescribe the "MetaClinic-home" app.
The chronic diseases that can be monitored by the "MetaClinic-home" app are: diabetes,
chronic obstructive pulmonary disease, and heart failure.
This system is not capable of managing clinical emergencies.
The application aims to highlight, among scheduled visits to specialist
centers, symptoms and clinical data detectable by the patient that may suggest
a worsening of the disease. The goal is to make
data and questionnaires available to the healthcare provider
according to a deadline established by the specialist for each patient. Symptoms
that are too often ignored by patients, which would be easy
to identify and which, if properly identified, could prevent
flare-ups of the same chronic diseases.
The system also allows for the exchange of messages between healthcare professionals and the
chronic patient. Unlike traditional chats, these messages are recorded
and highlighted in the METEDA Smart Digital Clinic medical record used by the
same specialists. Thus, the healthcare professional and the specialist
will be able to integrate the information received from the home with the patient's
complete medical history. This allows healthcare professionals to decide,
more carefully and precisely, the type of communication or request to make with respect
to the subsequent outpatient check-up of the patient undergoing monitoring.
The application is activated on the patient's phone at the discretion of the
specialist in the chronic disease being monitored and only in their presence,
thus avoiding problems identifying the person being monitored.
