Docket® is commissioned and directly endorsed by the following official government entities: Alaska Department of Health, Idaho Department of Health and Welfare, Maine Department of Health and Human Services, Minnesota Department of Health, New Jersey Department of Health, Utah Department of Health and Human Services, and Wyoming Department of Health.

Alaska, Idaho, Maine, Minnesota, New Jersey, Utah, and Wyoming residents with a valid phone number and/or email address on file with their state's immunization registry can access digital immunization records with the Docket® app.

Use of Docket® is entirely optional.

[10/22/24] I wasn't aware that only state immunizations were listed. Thank you for the correction. As of this writing, then, all my records are accounted for. [Original]The concept is good, but the execution is not there. My records are missing quite a few immunizations going back several years. As well as recently.

Very useful and nice to have all of this info in one spot. One issue - just got my flu shot and while it does show up, the dashboard still says OVERDUE in red. I'd also love to see an option to "turn off" the shots you don't have so they don't appear on the dashboard.

Entered details and clicked search. A squirrel (?) appeared for half a second and that was it. Tried again - same thing. Got a message that a search was already in progress. Gave up - no message ever shows . I tried this 4 times over 3 months with the same lack of app response. Today I got a result! Nope. Downloaded PDF was just general information from NJ.
