iHealth Screen was developed by Jockey Club CADENZA e-Tools for Elder Care. It was officially launched in August 2021 and upgraded to a new version in November 2024. This new version combines various functions such as health management, self-care education, and community resource repository. It allows older adults, caregivers, social and healthcare professionals to conduct 12 free self-health screenings for older adults, provide integrated health reports and corresponding educational resources, and find relevant community resources at anytime, anywhere through mobile phones or tablets.
The upgraded version of iHealth Screen introduces Hong Kong Elder Care Resource Map. Users can search for suitable medical and community services based on health needs, resource types, and locations. It also features a map interface and GPS navigation function to help users find their way to relevant service providers. Users can bookmark appropriate community resources and share them with friends and family through social media.
