Sensorium offers data services to detect and track the development of health risks and health needs of an individual or group of individuals at an early stage. The Sensorium app allows you to monitor various sets of health aspects in real time, providing healthcare providers with insight into the current health situation and health needs. Sensorium can be used for self-monitoring, remote care, patient panels, and population management.
It works like this: Your healthcare provider determines which health analysis and/or health needs assessment program is appropriate for you. They do this to help you improve your health and better tailor their services to your needs and those of others, now and in the future. Participation is currently only possible by invitation from your healthcare provider(s). The digital invitation from your healthcare provider contains a unique link that provides access to the program. After completing your registration, you can get started in the Sensorium app. From then on, Sensorium will help you identify patterns, identify risks, stratify, and classify the population using data analyses of your collected structured data.
The data collected by or from you is only accessible to you. Your data is also used to support population management. Sensorium processes this data so that it can no longer be traced back to an individual.
Once you indicate that you no longer want your data to be used for population management, all your data will retroactively be removed from population-based analyses.
