This application is designed, maintained and operated by Dr. Suzana da Câmara Tavares, (CRM-DF 26399). The insight came when she was not even a doctor yet, during the period of treatment for one of her children, who was suffering from a serious type of drug-resistant epilepsy. At that time, it was just a mother who faced the challenge of answering questions about the history of her son's illness, reporting the countless medications the child had used over time and which ones she was still using, presenting reports of dozens of tomography and magnetic resonance imaging scans, which were stacked in folders. Confirmation of the need for a tool that would help patients and their families manage information relevant to their own health occurred during clinical practice, through numerous situations that the doctor experienced during her care. The patients/family members did not know how to provide basic information such as the name of the medications, dosage, when they started such medications, what they were and when the diagnoses they said they had were made, whether there was effective control of chronic diseases, as they did not have laboratory tests available. recent although they had carried them out... They did not know when the screenings recommended by the Ministry of Health had been carried out, such as Pap smears and Mammography tests to screen for Cervical Cancer and Breast Cancer, or SOF (hidden blood in feces) and Colonoscopy to screen for bowel cancer. Finally, there was a lack of important data to frame the patient's real health status, data that would undoubtedly improve clinical practice.
