Once you create a personalized health profile, the app identifies factors that influence behaviors for each individual. It then creates a roadmap to improve long-term health and behavioral outcomes.

cant even proceed, the continue button is directly underneath the back button.... anytime I try to submit continue, it exits the app.

Every time I open the app it crashes. I've uninstalled it and reinstalled it numerous times and still the same thing. Please fix!

This app keeps crashing, i have install and uninstall several times and in diferent devices and it dosnt let you open it
