CareCorner is designed to support the day to day coordination of care for people with disabilities, people with chronic health conditions or elderly people.
CareCorner allows everyone to operate on the same page and stay up to date with the care recipient’s needs.
The agency or individual mainly responsible to coordinate Care for a person or the Care recipient themselves subscribes and sets up an account via our website (www.carecorner.net.au). They then invite contributors who can access the customised Care Platform via the App.
Agencies can sign up multiple clients at a time.
Nurses and Support workers can:
- review an up to date Nursing Care Plan.
- Add observations and interventions as the day goes on
- share progress notes with the whole team for a smooth shift handover
- review daily reports to ensure all care tasks are complete
- review custom reports to identify any trends
- easily access emergency contacts and plans
- sign off the administration of medications
- submit incident reports
- link the progress notes to goals
