About this app

First of all, what is a board?
A board is a theme-oriented, multidisciplinary patient file. Relevant medical data is shared in a board, treatment plans are drawn up and low-threshold communication and coordination takes place. A board also contains extensive options for telemonitoring.

cBoards, for all your collaborations!
The cBoards library makes a large number of different boards (care themes/paths) available that can be adapted to your own wishes. In addition, it is possible to completely compose a board yourself and to give it its own theme. cBoards can therefore be used for all necessary collaborations!
Examples of boards available in the cBoards library: GLI, Elderly Care, Medication Assessment, Diabetes Self-Management, Stroke, Heart Failure, Social Prescription, and Palliative Care.

For healthcare professionals:
You use cBoards to…
* set up teams consisting of colleagues and/or other healthcare professionals.
* create boards and collaborate on these around a patient, together with other (teams of) healthcare professionals, informal caregivers and the patient himself.
* register relevant medical data and share it with other board participants, so that everyone involved always has up-to-date and complete data.
* easy to communicate via chat and video calling with other participants in a board.
* share photos and relevant documents with other participants in the board.
* Easily communicate with your colleagues and other healthcare professionals via 1-on-1 and group chats, independent of a board.
* easy to draw up joint treatment plans.
* simple to define and assign tasks together, determine an evaluation service and monitor everything together.
* manage your patient population by creating lists and generating export files for analysis.

Integrations:
By linking cBoards with other care registration systems (including HISs, EPDs, ECDs, and hardware devices), data is exchanged back and forth. This prevents double registration charges.

* no local storage of medical data on your device, but completely safe in the Cloud.
