ImagineCare is an easy-to-use app for Remote Patient Monitoring, a digital healthcare service that makes it possible for patients to measure values regularly from home, with a direct connection to the healthcare staff.

The results from your home measurements will be seen immediately by both you and your caregiver, making it possible to get immediate support in case of health deviations.


This leads to, e.g.:

- A safer way to receive healthcare, wherever you are
- Increased and eased availability to healthcare staff
- Improved health self-control, with a good overview of your health and care plan


Further Information:
- For the best experience, install the app on a unit with a Bluetooth connection for automatic synchronizations with medical hardware. Connected measure equipment will then send data for analysis in safe systems with help from your Android unit.

- ImagineCare can optionally sync with Health Connect to provide a comprehensive view of your sleep and activity habits. If your healthcare team has chosen to monitor your Steps and Sleep – and you have approved the sharing of this data from Health Connect – the ImagineCare app can retrieve this information. This allows your healthcare team to better track your well-being and provide more personalized support.
