NOTE: The Limber Health Home Exercise app is only accessible and available to eligible patients enrolled by their provider.

The Limber Health Home Exercise app provides the tools to extend your care outside of the clinic and into your home offering:

FOLLOW ALONG VIDEO INSTRUCTION
On-screen demonstration and voice instruction helps you complete your prescribed home exercises with the proper form.

SESSION REMINDERS
Receive notifications to help you remember to complete your home exercise sessions.

PROGRESS TRACKING
Track your recovery progress with pain and function levels inside the app

AT-HOME SUPPORT
Have a question? If included in your program, you can chat with your remote Care Navigator right in the Limber app.


Getting Started with Your Limber Home Exercise Program is as easy as 1-2-3….

1. Download the app: Keep in mind that you won’t have access to log in until your provider enrolls you.

2. Complete Your Video Program: Through the app, you will receive guided instructional videos of the exercises prescribed by your provider. Simply press play and follow along!

3. Receive Support Every Step of the Way: If eligible, you will receive access to one-on-one virtual coaching from a Care Navigator, a licensed therapy professional, dedicated to providing:
- Virtual support in between visits
- Motivation and reminders to help you stay on track with your program
- Updates to your treating provider about your recovery outside of the
clinic.

My Limber Care Navigator and the PTs at Aquacare were wonderful. They listened to my concerns, commiserated with my pain reports, and provided support and encouragement throughout my pre- and post-surgery period. I didn't make use of the exercise videos in the app, but it was nice to know they were available. The progress questionnaires could be better worded, but overall the app was a great tool. Hearing from my Care Navigator regularly via the chat feature in the app was very much appreciated.

This is excellent in helping me maintain my PT exercises. The only drawback is that I can't directly contact my PT Doctor. He needed to adjust one of my stretches as it wasn't long enough. There should also be a reminder of what apparatusrs are needed to do the exercises.

Depictions and demonstrations are very specific and clear. The automated countdown for each exercise is convenient. However, for progress reports, with some injuries/surgeries certain activities are prohibited, such as, jogging or running is contraindicated for me following total knee replacement.
