eCareMD is a chronic care management platform designed to keep patients connected with their care teams. The eCareMD Patient App helps patients stay engaged with their health programs, communicate with care managers, and improve adherence to programs such as CCM (Chronic Care Management) and RPM (Remote Patient Monitoring).
