About this app

Health Connected is a digital health app that helps you understand and manage your wellbeing. It combines validated tools and insights to identify potential health risks, track lifestyle habits, and access trustworthy health information - all within a secure environment.

Access to Health Connected is provided through participating extended benefits programs. If you’re unsure whether you have access, please contact your health benefits or wellness program administrator. You’ll need to enter your client code to activate and access the app’s features.

Key Features

• Health Scan: A 30-second facial scan that can be completed from a desktop, tablet, or mobile camera. Health Scan analyzes facial indicators with 95–99% reliability to assess a range of health indicators related to physical health, mental wellbeing, and potential health risks - all without the need for cuffs, sensors, or other devices.

• Health Risk Assessment: A questionnaire that helps identify potential health risks based on your responses, lifestyle, and daily habits. Your results include an Adjusted Age, which compares your current health status to your biological age, offering insight into how your choices may be influencing your overall health.

• Health Skills Profile: Learn about your health knowledge, confidence, and daily practices, with suggestions for improvement.

• Personalized Guidance and Navigation: View recommendations based on your results and health profile. The app will direct you to relevant resources available through your health benefits package, as well as publicly available supports and services.

• Challenges: Participate in individual or team challenges to help build healthy habits.

• Digital Health Coach: Receive coaching to support your wellness goals.

• Trackers: Sync data from your phone and other devices to Google Health Connect to record steps and sleep.

• Health Records: Organize your personal and family health history in one secure location.

• Health Library: Explore validated information on medications, conditions, and wellness topics.

Access and Use
Health Connected is available through select extended benefits programs. To activate the app, enter your client code. If you’re unsure about your access, contact your health benefits or wellness administrator.

Google Health Connect Integration

Health Connected uses Google’s Health Connect to make it easier for you to access and use key tracking features within our platform.

With your permission, Health Connected reads only the following data as needed:

• StepsCadence/Steps: Automatically sync your daily and historical step counts to your Trackers profile. Your steps are also linked to Virtual Adventures challenges, so your progress updates automatically — no manual entry required.
• Sleep Sessions: Automatically sync your sleep data to your Trackers profile to view your current and past sleep patterns at a glance.

Health Connected never collects more information than necessary and always requires your explicit consent before accessing any data.
