The UHD project runs on a platform that allows remote interaction with patients, regular monitoring of their health status, prioritization of interventions depending on the evolution of the clinical situation and intervention at patients' homes whenever necessary.

The offer is structured in “clinical pathways” that define the optimized approach to the clinical situations that we are going to serve. These “pathways” are then translated into individual care plans that reflect each patient's personal and unique needs. These multidisciplinary plans allow the proper and timely execution of acts and the recording of relevant clinical information and generate an alarm for professionals and patients to act at the right time and in the right way.

This approximation of the patients' clinical situation allows an overall view of their situation and the maintenance of a clinical history that supports reasoned and timely clinical decisions. All this at home, without unnecessary travel.
