Sereniq is a mobile-first documentation platform built for Texas IDD (Intellectual and Developmental Disabilities) group homes and HCS / HCBS providers. It replaces paper binders and dated desktop systems with fast, guided, audit-ready record-keeping that direct-care staff can complete from their phone, even with no signal.
Sereniq is for licensed care agencies. Accounts are created by an agency administrator; there is no public sign-up, because the app handles protected health information.
WHAT YOUR TEAM CAN DO
• Clock in and out, with on-site location verification for each assigned home
• Write shift and progress notes, structured for HHSC Form 4119 (group homes) and Form 4122 (host homes), with optional AI assistance that turns quick notes into clear, person-first narratives
• Record medications on the MAR (Form 3092), with taken, refused, home-visit, and hospital states
• File incidents, injuries, allegations, and medication errors; routed to supervisors for review
• Track care plans, appointments, transportation logs, fire drills, and grocery runs
• Get push alerts for note reviews, incident events, and upcoming credential and care-plan expirations
BUILT FOR REAL SHIFTS
• Offline-first: notes, MAR entries, and incidents save on the phone and sync automatically when signal returns
• Simple, icon-led screens designed for fast use on low-end devices
• Available in English and Spanish
BUILT FOR COMPLIANCE
• Designed around HHSC documentation requirements so records stay ready for state audits
• Protected health information is encrypted in transit and at rest
• Every action is access-controlled and audit-logged
• Role-based access for Direct Support Professionals, house managers, nurses, supervisors, case managers, and administrators
Sereniq is a private tool for care agencies and their staff. To use it, your agency must have a Sereniq account.
