About this app

Click Check+ (click Check Plus) to establish a new model of remote home health services for participants. Combining technology and medical teams, health data such as blood pressure, blood sugar and heart rate are integrated through the Check+ smart app. When there are any changes in the health data of participants, they can receive professional advice and support from the central nursing team.​ Service target: Tuen Mun District Health Center members Service content: Once or twice a week, nurses will monitor participants' conditions and various data trends in the system. If any abnormality is found, the nurse will understand, follow up and make suggestions to the participants via phone or SMS. Service provision period: 1-12 weeks (depending on the needs of the participants). Tuen Mun District Health Center combines an inter-professional team composed of nurses, allied medical staff, pharmacists, nutritionists, social workers and support staff, and is equipped with intelligent health Management system to provide citizens with various government-funded primary medical services.
